A medication authorization form must be filled out for all medication, both prescription and over-the-counter. The form must be filled out entirely and signed by a physician and parent or guardian for a student to take the medication in school. Only emergency medications will be permitted to be carried by the students and, in order to do so, must be signed off on the appropriate section of the form. All other medication must be dropped off in the nurse's office where it can be counted and documented as received. Once a medication is brought in, students may go to the nurse's office as needed to receive their medicine.
